Abstract

Stanford's Chronic Disease Self-Management Program (CDSMP) stands out as having a large evidence-base and being broadly disseminated across various countries. So far, there has existed neither evidence nor practice of its systematic adaptation for German-speaking countries like Switzerland and Austria. The objective of this paper is to present findings from an evaluation that was built into the adaptation and implementation process of the CDSMP in this area. The study was conducted in 2010-2013 to determine whether the adapted version of the CDSMP could be an asset to existing chronic care strategies in French- and German-speaking central European countries.
